my computer states that i have sevea; malware issues..

My issue is that my computer has been running VERY slowly , and it takes a half an hour or get the internet to come up ..( when it does) ... Now i maintain my grandparents computer to the best of my ability , but i don't understand a lot of the technical terms ... I do not have Hijackthis because i have been warned that it can do more harm than good ... the thing that annoys me most is the fact that all but the wild tangent appear to be in registry ( according to spybot)

please help me understand and get it running smoothly again,
