Run Once On Startup - Advise Please

When I select the option from setup - Run Once On Startup .... it does not run as expected on startup..

It's as if the program does not recognise the option as selected.
1. On Restart
Or
2. After a shutdown and power up.

Oddly
If Spybot prompts the user to Run Once On Startup after a scan it restarts and runs before anything else... on startup.

Question
How do I get Spybot to Run Once On Startup before anything else everytime I select the option in the settings menu.

Spybot does not seem to recognise the option everytime it has been selected.
I can't figure out why it works sometimes but not everytime I choose the option.
I've had this issue for years but never asked till now.

Hello,

Please open Spybot-S&D with a rightlick and choose "run as administrator/take ownership".
Then go to "Settings" -> "Settings" and scroll down to "System start".
Here click the radiobutton "Run program once at next system startup" AND the checkbox in front of "Run check on program start".
