There are two things that you can do:
Run Spybot the next time you reboot (this will run Spybot before most other processes start):
Go into Spybot > Mode > Advanced mode > Settings > Settings > look for "System start" (located half way down the page).
Check the option: "Run program once at next system startup".
Exit Spybot.
Reboot the system.
Run Spybot in Safe mode .
Reboot your system in Safe mode (Instructions if needed - Starting your computer in Safe mode (http://service1.symantec.com/SUPPORT/tsgeninfo.nsf/docid/2001052409420406?OpenDocument&src=sec_doc_nam)).
Run Spybot-S&D.

In addition, if Brook is inquiring about running any application, (and not specifically Spybot-S&D) I would suggest reading information at sites such as:

http://www.kellys-korner-xp.com/xp_tweaks2.htm

http://www.tweakxp.com/

http://www.dougknox.com/

http://www.microsoft.com/windowsxp/downloads/powertoys/xppowertoys.mspx

Especially as you are speaking of manipulating the registry. ;)
