Persistent.

Hi everybody, I need some help and advice to rid myself of a persistent malware infection. Spybot picks it up every time and looks as if it has removed it but it always shows up in the next scan.
I have run - Adaware, Spybot, AVG, Superspyware blaster, smitfix, vcleaner, SDfix, and Fagobot.
I thought I had succesfully removed it but its back again, I'm obviously missing something.
All help or advice will be greatly appreciated.
