Welcome to Safer Networking, I wish to be sure you have viewed and understand this information.
"BEFORE you POST" (READ this Procedure before Requesting Assistance)
http://forums.spybot.info/showthread.php?t=288
All advice given is taken at your own risk.
Please make sure you have read this information so we are on the same page.
You are infected, I suggest you keep this computer offline except when troubleshooting, the junk may download more. If you have any tool I use, delete it and download it new from the link I provide. Read and follow the directions carefully, the tools will not work unless you do.
Before we start, you have what are probably infected emails stored and I would not know they were there without the Kaspersky Scan. The tools I use will not touch those and I suggest you delete them manually. I can piont you to them and you might be able to use your resident antivirus program to delete them or at least scan them for infections. This is where they are located:
C:\Data\Outlook\outlook.zip
C:\Old Machine\Data\Dell Laptop\Backups\All_20031013\E_Data_20031013.zip
C:\Old Machine\Data\Dell Laptop\Backups\C_20050123\Outlook (docs and settings_JC_local settings_app data_microsoft).zip
C:\Old Machine\Data\Dell Laptop\Backups\E_Outlook_20031013.zip
C:\Old Machine\Data\Dell Laptop\Backups\Outlook 20030412.zip
C:\Old Machine\Data\Outlook\outlook.zip
Moving on to your infection, I need to do some research like this:
Kaspersky indicates these files are infected, please scan them with one or more of the tools I provide:
C:\Documents and Settings\John\LetsPowwow\lib\DskHooks.dll
C:\Documents and Settings\John\LetsPowwow\lib\VNCHooks.dll
C:\Software\VNC\vnc-E4_2_6-x86_win32.exe
C:\Software\VNC\vnc-E4_2_6-x86_win32.zip
http://virusscan.jotti.org/
http://www.kaspersky.com/scanforvirus
http://www.virustotal.com/
John, if you find them bad, deleted them, and just let me know.
http://siri.geekstogo.com/SmitfraudFix.php <<< download Smitfraudfix from here and follow ONLY these directions.
Search:
Double-click SmitfraudFix.exe
Select 1 and hit Enter to create a report of the infected files. The report can be found at the root of the system drive, usually at C:\rapport.txt
Note: process.exe is detected by some antivirus programs (AntiVir, Dr.Web, Kaspersky) as a "RiskTool". It is not a virus, but a program used to stop system processes. Antivirus programs cannot distinguish between "good" and "malicious" use of such programs, therefore they may alert the user.
http://www.beyondlogic.org/consulting/processutil/processutil.htm
Post only the C:\rapport.txt
post any information I requested also.
Thanks

I manually deleted all 6 of the old email files you identified in your post.
Kaspersky virus scan found the LetsPowwow DLL's you identified to be infected and I manually deleted them, then deleted the entire folder they were in. The VNC files were too large for Kaspersky to do a browser scan, so I just deleted them, also, to be safe.
Recycle bin has been emptied after the above steps...
Thanks for your help!
Here's the Rapport.txt:

Thanks for returning your information, Smitfraudfix found the infection and it also found this:
»»»»»»»»»»»»»»»»»»»»»»»» hosts
hosts file corrupted !
After we clean, in the next C:\rapport.txt, there may be a very large hosts file (items starting with 127.0.0.1) and I do not need to see it. Edit (remove) it from the C:\rapport.txt before you post it.
Clean:
Reboot your computer in Safe Mode (before the Windows icon appears, tap the F8 key continually)
Double-click SmitfraudFix.exe
Select 2 and hit Enter to delete infect files.
You will be prompted: Do you want to clean the registry ? answer Y (yes) and hit Enter in order to remove the Desktop background and clean registry keys associated with the infection.
The tool will now check if wininet.dll is infected. You may be prompted to replace the infected file (if found): Replace infected file ? answer Y (yes) and hit Enter to restore a clean file.
A reboot may be needed to finish the cleaning process. The report can be found at the root of the system drive, usually at C:\rapport.txt
Optional:
To restore Trusted and Restricted site zone, select 3 and hit Enter.
You will be prompted: Restore Trusted Zone ? answer Y (yes) and hit Enter to delete trusted zone.
Note, if you use SpywareBlaster and/or IE-SPYAD, it will be necessary to re-install the protection both afford. For SpywareBlaster, run the program and re-protect all items. For IE-SPYAD, run the batch file and reinstall the protection.
Post the C:\rapport.txt and a new HJT log.
Thanks

I love the way your process works - the directions are clear and it feels like we are making good progress - thanks for your help!
Here's what I found in following your directions:
1. I double-clicked SmitfraudFix.cmd (no .exe, so you may want to update that detail in your directions) and cleaned the registry.
2. I was not prompted about wininet.dll (I assume it was checked and nothing was found).
3. It was curious to me that a "Disk Cleanup" process was launched somehow (to see how much space could be saved by using compression on drive C:) - I cancelled it.
4. The Safe Mode dialog box reappeared at this point, but no reboot was requested/forced.
Here is the rapport.txt (edited to remove all but one of the 127.0.0.1 entries):

Thanks for the feedback, let's do some cleaning like this:
Please download ATF Cleaner by Atribune
http://www.atribune.org/public-beta/ATF-Cleaner.exe
Save it to your Desktop. We will use this later.
You may use clean manager instead of downloading ATF-Cleaner if you wish. It will compress files for you like you mentioned above.
http://spyware-free.us/tutorials/cleanmgr/ <<< here
Open HijackThis and choose "Do a system scan only" then check the box in front of these line items:
R3 - URLSearchHook: Yahoo! Toolbar - {EF99BD32-C1FB-11D2-892F-0090271D4F88} - (no file)
O2 - BHO: (no name) - {7E853D72-626A-48EC-A868-BA8D5E23E045} - (no file)
O3 - Toolbar: etlrlws - {FB764024-567A-43E8-8345-770862932DEB} - C:\WINDOWS\etlrlws.dll (file missing)
(next is optional, see this: http://www.castlecops.com/startuplist-180.html )
O4 - HKLM\..\Run: [AlcxMonitor] ALCXMNTR.EXE
Close all programs but HJT and all browser windows, then click on "Fix Checked"
Run ATF Cleaner
Double-click ATF-Cleaner.exe to run the program.
Click Select All found at the bottom of the list.
Click the Empty Selected button.
Click Exit on the Main menu to close the program.
If you have no issues with those instructions, run a new Kaspersky scan using these settings:
* The program will launch and then begin downloading the latest definition files:
* Once the files have been downloaded click on NEXT
* Now click on Scan Settings
* In the scan settings make that the following are selected:
* Scan using the following Anti-Virus database:
* Standard
* Scan Options:
* Scan Archives
* Scan Mail Bases
* Click OK
* Now under select a target to scan:
* Select My Computer
* This will program will start and scan your system.
* The scan will take a while so be patient and let it run.
* Once the scan is complete it will display if your system has been infected.
* Now click on the Save as Text button:
* Save the file to your desktop.
Then post it here. How is the computer running?
Thanks

I completed the fixes you recommended using HijackThis (including the optional one - thanks!).
I ran the ATF cleaner as instructed (freeing 1.8Gb).
The computer is running much better, now! It would not be an exaggeration to say it was "near death" and appears to have made a full recovery! I can't thank you enough.
As the latest Kaspersky indicates, though, I still have some infections. They all appear to me to be in my Outlook-related files and backups. I don't mind getting rid of some of those backup files (I'd be happy to delete every file in the "Old Machine" folder), but it would be nice to be able to keep some of the PST files in the "C:\Data\Outlook" folder if possible. Some of them mention specific items in the PST file - is it an option to delete just those items? Of course, my main priority is to "get clean" so I will do whatever it takes to do that.
I look forward to your recommendations on how to proceed.
Here is most of the latest Kaspersky log (does it take nearly 7 hours to run on other machines?) - I had to delete some of it to get under 20000 characters (I removed only items in the "C:\WINDOWS\" and "C:\Program Files" directories and subdirectories, all of which indicated "Object is locked skipped"). If you want the whole file, just let me know how to send it.

I'll comment on your feedback prior to looking at the KOS.
Folks are usually surprised at how much space ATF-Cleaner frees up. Keep in mind the initial instructions include cleaning Prefetch:
http://www.windowsnetworking.com/articles_tutorials/Gaining-Speed-Empty-Prefetch-XP.html
and that will slow your boottime until Windows repopulates the folder with needed files. I clean Prefetch on my computers only when I look and see it needs it. We do it during this process because hackers have learned to hide their junk there.
You have a lot of files to be scanned:
Total number of scanned objects: 268837
I have about 1/4 of that amount on my four year old Dell/WindowsXPPro and it takes about 1 hour to scan a clean computer.
You might want to take a hard look at why you are storing so many files, they are surely not all Windows files.
KASPERSKY ONLINE SCANNER REPORT Wednesday, March 19, 2008 1:26:35 AM
Number of viruses found: 5
Number of infected objects: 26
Number of suspicious objects: 10
All of these items expect three which are infected System Restore files are located here:
C:\Data\Outlook\
C:\Old Machine\Data\Dell Laptop\Backups\
C:\Old Machine\Data\Outlook\
My scanner isolates them so I can see them but there are a lot. I would be glad to attach the file for you if you wish.
You can clean the three System Restore files with this information:

I will post this information for you so you can benefit from it. If I can do more, let me know.
