BHO added at startup

Spywareguard keeps telling me that a BHO has been added everytime I startup the pc, I tell it to remove it and it says it has done but straight away pops up again to tell me the BHO has been added again.
The last couple of times it has said the BHO has been added by efcATMcd.dll in c:\windows\system32\
I have tried booting in safe mode in order to delete this .dll but i can't as it says it is in use. I have also noticed another .dll that has appeared since this problem started hgGxVMCv.dll and i cannot remove this one either.

I have tried various spyware/adware removers but none seem to find a problem.

UPDATE

managed to delete efcATMcd.dll using vundofix, however when i rebooted spywareguard reports a new BHO, this time created by fccbyxvw.dll so it would seem if 1 is deleted another is created to replace it.

also it seems that these BHO's are downloading a virus in the background because AVG is now reporting LOP virus in the temporary internet files folder even when i have cleared the folder and not run IE

1. Download combofix from any of these links and save it to Desktop:

**Note: It is important that it is saved directly to your desktop**

2. Double click combofix.exe & follow the prompts.
3. When finished, it shall produce a log for you (C:\ComboFix.txt). Post that log & a fresh hjt log in your next reply

Note:
Do not mouseclick combofix's window whilst it's running. That may cause it to stall

Combofix should never take more that 20 minutes including the reboot if malware is detected.
If it does, open Task Manager then Processes tab (press ctrl, alt and del at the same time) and end any processes of findstr, find, sed or swreg, then combofix should continue.
If that happened we want to know, and also what process you had to end.
