Alfacleaner won't go away (grrr!!!)

I got infected with the Alfacleaner trojan, and managed to clean most of it out (Ad Aware, Spybot, Hijack This, SmitRem). However, a couple of issues remain.

(1) Everytime I run Ad Aware, it catches the Alfacleaner registry entry, and I remove it. It always comes back.

(2) When I open IE for the first time, a Command box immediately opens up and starts to run Setup.exe in the following folder:

c:\Documents and Settings\Joe Blow\Local Settings\Temp\

Any ideas?
