Persistent Smitfraud and Virtumonde

Hi,
Have been helping a friend try to clear his PC. Have run Spybot multiple times - various flavours of Virtumonde and Smitfraud seem to remain each time.
Can't run windows update, can't update Mcafee.
System will only boot in safe mode, and even then not every time.

Have followed "Before you post..." steps - disabled teatimer, backed up registry with erunt.

Would be extremely grateful for assistance.
