some basic malware/spybot questions

hey guys
wondering if anyone can answer a couple questions that have always bugged me;

whats the basic steps to remaining clean/safe online? (is it possible ?!)

how come when i do a system scan in spybot it finds one or two infections but dosent alert me when those infections 1st arrive on my system?

my A.V software seems to regularly alert me of the same infections but when i try to heal them the 'infected file doesnt exsist'?!
