For what it's worth, I did find these 2 entries in the OTLPE log suspicious- they are the most recent files shown, and the date/time...

Running chkdsk with the p switch was a good move, but it may be necessary to probe a little deeper.
Try running chkdsk again but this time with the r switch
chkdsk /r
allow plenty of time for this...
