Can't kill winfixer

Any hints on getting winfixer to go away? Spybot finds a couple of winfixer cookies, but that's not enough. It comes back. Am I doing something wrong?

Have you the latest Spybot-S&D definitions 11-18-05?

As we are seeing a fair amount of winfixer infections please go here and follow instructions.
