There should be no compatibility issues with Spybot-Search&Destroy and iTunes. I have iTunes (with TeaTimer active).
What I did realize from my observation is that iTunes will take some seconds to bootup, and it might "lag" a little if you have a iPod plugged in.
With 512MBs of RAM and 33 processes, iTunes takes a good 7-12 seconds to load on my PC.
