Unable to add domains to settings tab

My settings tab looks like this:



How do I add Ok domains if there is no add button?

I have never seen it because I don't have the proxy enabled, but I would presume that when the proxy blocks a domain, you are given a prompt by Spybot which asks if you would like to whitelist the domain.
