I was not able to see the image you refer to, but I think it is probably similar to the one I have attached to this post.
When the current version of Spybot starts up, it checks the number of temporary files at c:\Documents and Settings\Username\Local Settings\Temp (at least that's for XP), and if there is more than I believe the figure is 100 it displays that dialogue. The number value in the left hand button is a countdown.
Normally if you click Yes it will clean out (delete) the temporary files, however in your case it seems they are "in use".
If you answer No, it will continue to start up the Spybot program.
It may be worthwhile doing an Antivirus scan of the Temp folder, and also a full Spybot Scan to see whether any of these files are part of an infection.
None of the files in that location should be other than temporary so you could try manually deleting them yourself.
Otherwise could you please advise what operating system you have, what antivirus software you are using and whether you have any other antispyware software installed.
Edit: The thought occurred to me that if you had been installing any new software before you started Spybot, and had not rebooted in between, some of these may have been part of the installation process and still "in use".
