I've noticed that sometimes programs get on my exclusion list that I didn't add.
- How did this happen?
- Is there a way to clear the exclusion list completely?

Programs? Can you be a little more explicit (Ignore products maybe)?
- Ignore products
- Ignore cookies
- Ignore File extensions
- Ignore single entries
- Ignore System Internals
What are the specific entries that are being ignored (there are certain items ignored by default when you install Spybot)?
I don't know how that could happen because the ignore lists are protected and any attempt to change them outside of Spybot itself causes that ignore list to be cleared. Unfortunately this even occurs if you switch between/among users in a multi-user environment.
Yes. If you are talking about "Ignore products" for example, go into Spybot > Mode > Advanced mode > Settings > Ignore products. Click on the "All products" tab then right click on the list of products and select "Deselect all".

See if the following discussion answers your question:
- Why are CDilla & SideStep checked in Ignore Products?
