When you start Spybot-Search&Destroy you'll be presented with options such as:
Spybot does not scan file by file on the computer as it will take too long. How Spybot scans is briefly described in the FAQ below:

How Spybot protects you is using Immunization (Passive protection against malicious sites) and TeaTimer (Resident Shield) that actively disables known malware. TeaTimer (v1.6.x) currently has a third of a million malware definitions (TeaTimer will stop them).

Can someone explain what the Immuniser does please? (Post#3) by DougCuk
The update option is pretty straightforward as we will just have to click on it, update to the latest definitions from a update mirror and then we're done. Usually following each update is an update to the Immunization.

If renaming the desktop icon of Spybot does not work (malware is blocking the execution of Spybot) try this thread by md usa spybot fan:

If you removed something by accident and want to recover it from Quarantine it is always available in the "Recovery" tab in the Main Screen.
